Blood Pressure Log: A Simple Format Your Doctor Can Use
A blood pressure log is a written record of your home readings, and it works best when every line holds the date, the time, two readings, your pulse and a short note. The AHA asks you to record results and bring them to appointments. Keep it to one page a week so your doctor can scan it fast.

01The format
What should a blood pressure log include?
A useful blood pressure log has eight columns: date, time, systolic, diastolic, pulse, arm, medicine taken and a note.
The AHA says to record your results and bring them to appointments (AHA, Home blood pressure monitoring). A clean format saves the visit time.
Each line is one reading. Two readings per session means two lines, one minute apart.
| Date | Time | Sys | Dia | Pulse | Arm | Medicine | Note |
|---|---|---|---|---|---|---|---|
| Oct 20 | 7:10 am | 128 | 79 | 68 | Left | Not yet | Slept 6 h |
The sample line is a format example, not a typical reading.
02Useful notes
What notes make a log more useful?
Short notes about sleep, salt, stress, caffeine and new supplements explain the spikes before your doctor has to ask.
Numbers alone show what happened. Notes show why. Keep them to a few words.
Before the reading
Coffee at 8, walked the dog, rushed to the chair.
Daily context
Salty dinner, poor sleep, hard day at work.
What you take
Prescriptions, and any supplement with its start date.
Write the supplement start date exactly. If you add Melonex, copy its caution panel into your notes and show your doctor the bottle. See the 30-day check-in for how to compare before and after.
03Read the week
How do you read a week of readings?
Average the morning readings, average the evening readings and look at the highest and lowest line.
Add the systolic numbers for one slot and divide by how many you took. Do the same for diastolic. That is your average for the slot.
A single high line is not a finding. A pattern is. Mayo Clinic suggests at least two sessions a day when you begin (Mayo Clinic, Blood pressure: Can it be too low or high). Our post on the best time to check explains the slots.
Compare your averages with the AHA categories: normal is under 120 and under 80, stage 1 starts at 130 or 80, stage 2 at 140 or 90 (AHA, Blood pressure readings). Only a clinician diagnoses.
- Circle the highest line each week
- Mark days with poor sleep or stress
- Bring the sheet, or a photo of it
04When to act
Which readings in a log need a call?
A reading above 180 and/or 120 needs a recheck after one minute, and symptoms with it mean 911.
The AHA says to wait a minute and measure again. If it stays that high with no symptoms, contact your health care professional. With chest pain, shortness of breath, back pain, numbness, weakness, vision change or trouble speaking, call 911 (AHA, Blood pressure readings).
The other end counts too. Pressure lower than 90/60 can bring dizziness, fainting, blurry vision or confusion (NHLBI, Low blood pressure).
- Dizzy or lightheadedEspecially when you stand up
- FaintingOr feeling you might
- Blurry visionComes and goes
- Weak or confusedUnusual tiredness
- Pounding heartbeatPalpitations, nausea
90/60Below this is low blood pressure (NHLBI). If these signs start after adding a supplement to blood-pressure medicine, sit or lie down, stop the supplement and call your doctor. Do not change your prescribed medicine on your own.
180/120Above this, wait a minute and measure again. Still that high with chest pain, shortness of breath, back pain, numbness, weakness, vision change or trouble speaking: call 911.
Low-pressure signs from NHLBI (NHLBI, Low blood pressure); the 180/120 steps from the American Heart Association (AHA, Blood pressure readings).
Keep taking prescribed medicine and tell your doctor about low or high lines in the log. Do not change a dose on your own.
05Keep it going
How do you keep a log going for weeks?
Tie the log to a daily anchor, keep the sheet next to the cuff and keep the format identical every week.
The cuff on the kitchen counter beats the cuff in a drawer. Put the log beside it.
Pair the habit with walking and calmer evenings; see walking for blood pressure and stress and blood pressure.
A Melonex bottle holds 60 capsules, about 60 days at one a day. Read the label first. It is a supplement, not a treatment, and no trial tested it.

Prices listed on the seller's order page; confirm at checkout. The 2-bottle kit costs $158, with $9.99 shipping. Ask your doctor about your medicines first. 60-day money-back guarantee per the seller.
Melonex is not a treatment; it is a dietary supplement. Ask your doctor or pharmacist first if you take blood-pressure medicine, nitrates for chest pain, an erectile-dysfunction drug such as sildenafil or tadalafil, a potassium-sparing diuretic (amiloride, spironolactone, triamterene), a blood thinner, or diabetes medicine, or if you have had a recent heart attack. Arginine and citrulline can add to these. Keep taking prescribed medicine. See side effects and interactions.
06FAQ
Melonex questions people ask
What is a good blood pressure log template?
Use eight columns: date, time, systolic, diastolic, pulse, arm, medicine and note. One line per reading, two readings per session. A paper sheet, spreadsheet or notes app all work as long as the columns never change.
How long should you keep a blood pressure log?
Keep it for at least a week before a visit, and longer if your doctor asks. Many clinicians want two sessions a day. Keep logging after a medicine or lifestyle change so the before and after are on the page.
Should you record the pulse in a blood pressure log?
Yes, most monitors show it, so copy it down. It costs nothing and gives your doctor extra context. Do not read much into a single pulse number yourself.
Can you use a phone app for a blood pressure log?
Yes, if it exports a clear table with date, time, both numbers and notes. The AHA only asks that you record results and bring them. A printed copy is a safe backup for the exam room.
